Year 9 DTP & Meningitis ACWY Vaccination
Dear Parent/Carers
We are visiting your child's school to administer the Year 9 Diphtheria, Tetanus, Polio and Meningitis ACWY Vaccinations on 13th March 2024. If you haven't already please use the link below to give consent for your child to receive these vaccinations when we attend, please note the link for consent will close 48 hours prior to our visit
If you do not wish for your child to receive these vaccinations we kindly ask that you use the same link and complete a 'No' consent to avoid us contacting you further.
